Payroll Financing Has to Reach the First Paying Work

A clinic has demand for more appointments but must credential two clinicians before they can bill. Payroll will arrive every two weeks while patient collections trail the visit. The decision is how many hires the current clinic can carry until the new schedules pay.

The first six weeks have no new collections

Suppose both clinicians together cost $5,000 each pay period including employer expenses. A six-week credentialing period creates three pay runs, or $15,000, before their visits begin. Training, support staff, and rent add to that amount. Claims then need to be submitted and paid, so the true cash trough extends beyond the first appointment. Place each pay date and expected collection week alongside current clinic cash and existing debt.

If one clinician can begin serving patients sooner, a staged hire may reduce that trough. A signed service agreement is useful evidence of demand, but its revenue belongs in the forecast only as patients are seen, claims accepted, and money collected.

A denial or delayed credential can move the payoff

Test another month without the second clinician's billable visits and an allowance for denied claims. The clinic still pays both employees and must maintain existing care. If the current practice cannot cover that case, reduce the hiring pace or negotiate a later start rather than borrow to pay wages indefinitely. Repeated payroll borrowing after the new schedule is full signals that reimbursement or staffing margin needs repair.

Keep the bridge tied to its exit

A working-capital facility might bridge a documented timing gap under actual provider terms, but a fixed payment that starts during credentialing can worsen the earliest weeks. Compare repayment frequency, total cost, and a credible paydown from collected visits.
